The majority of urological complications are related to the ureteroneocystostomy, and the initial treatment for leakage or stenosis of the anastomosis is placement of a percutaneous nephrostomy (PCN) drain. Ureteral stents can minimize the number of urological complications, with two types of ureteral stents used: an internalized Double-J stent and an externalized Single-J stent. No well-designed prospective randomized controlled trial is available, as previous studies were retrospective in nature.\nMethods: We performed a single center randomized controlled trial. Patients were randomized to either an externalized Single-J stent, removed after 9 days, or an internal Double-J stent removed by cystoscopy after 3 weeks. Primary outcome was PCN placement within six months after transplantation, thereby assessing whether Double-J stenting was superior in preventing urological complications. Secondary outcomes encompassed urinary tract infections, cost-effectiveness, and hospital admission time.\nResults: The study was conducted from November 2018 to August 2023, during which 300 recipients were included with complete follow-up. PCN was performed in 14.5% in the Single-J group (21/145) and 4.5% in the Double-J group (7/155), p = 0.003. Multivariable logistic regression, corrected for recipient age, BMI, sex, and donor type, showed an OR of 0.26 [0.10, 0.61] (OR [95%CI]).
